Performance Evaluations
You will be scheduled for a performance evaluation every 3 months.

 3 & 9 Month Reviews
These reviews are a general check-in. Your supervisor will go over your performance, tell you if you're on track or not, and answer any questions or concerns you may have. 

6 & 12 Month Reviews
Employees are eligible for a raise at these reviews. Raises are dependent on performance and the scores received on the Performance Evaluation Form.

You will fill out this check-in form before each performance evaluation. It helps us understand where you are at and how you feel.
